Previous studies show that severe intermittent hypoxaemia, quantified by the duration, depth and frequency of oxygen desaturations, is associated with daytime sleepiness and cognitive decline in OSA patients [7–10]. However, desaturation metrics and other parameters quantifying the severity of OSA are not fully capable of explaining the impaired performance in PVT in OSA patients having shallow and short desaturations . This suggests that additional factors could explain the associations between OSA and poor PVT performance.

Therefore, novel biomarkers could be extremely useful for patients with relatively stable nocturnal oxygenation despite repetitive breathing cessations. Conclusions In conclusion, increased APF in PPG provides a possible polysomnography indicator for deteriorated vigilance especially in male OSA patients. This finding highlights the connection between cardiorespiratory regulation, vigilance and OSA.

The cells are ordered by their discharge probability during γ- or β-oscillation (20-ms bin size). A large proportion of FS cells show an increased firing rate during γ and β. Consistent with this irreversible neural damage model, previous reports showed that the severity of REM atonia loss is enhanced as the disease duration increases (Iranzo et al. 2009; Postuma et al. 2010). One explanation for the discrepancy is that RBD symptom frequency could be discordant with the severity of REM atonia loss. To conclude this assumption, a future prospective study with follow-up PSG will be required.

To investigate whether the decreased PDF levels seen at the dorsal projections are because of decreased PDF production or to a failure to recruit PDF-loaded vesicles (i.e., transport) toward the axonal terminal, we measured PDF levels in the sLNv somas. We analyzed somatic PDF levels and found that PDF immunoreactivity cycles in the sLNv somas in a way that resembles its cycling at the axonal terminals, with more PDF during the early morning and less PDF at the beginning of the night (Fig. 6B,E,F). Interestingly, in the context of Ih downregulation, somatic PDF shows an abnormal accumulation during the night, which could be because of a decreased daytime transport toward the axonal terminals that results in anti-phase cycling of somatic PDF levels.

However, differences in PPG features diminished in female patients and were not as clearly distinguishable. Stepwise regression analysis revealed that higher APF and t90% are associated with a higher number of lapses in PVT. These results imply that increased APF together with more severe nocturnal hypoxaemia may provide a PSG marker for impaired vigilance in male OSA patients. In addition, findings are in line with previous studies, indicating that female sex and older age are independent risk factors for poor PVT performance. Together, the collective evidence indicates that Ih may be modulating circadian rhythms and sleep by exerting its role in more than one neuronal type. Whether the molecular mechanisms that regulate and are regulated by Ih in LNvs and in Deep Sleep Music dopaminergic neurons are similar will require further examination.
